Residential Services
The bridge provides transitional and permanent supportive behavioral health housing to 1,369 single adults in three boroughs (manhattan, the bronx, and brooklyn). Individuals are housed in a combination of single-site bridge-owned buildings and scatter-site apartments rented from private landlords in the community.
Community-Based Services
(1) care coordination: the bridge is under contract with two health homes and works with approximately 900 health home enrolled members in the bronx, brooklyn, queens, and manhattan providing outreach, engagement, and ongoing care coordination.
Clinical Services
The bridge operates an office of mental health (omh) licensed personalized recovery oriented services (pros) program, which is a psychiatric rehabilitation program. Pros supports clients through group-based psychiatric rehabilitation services in a person-centered, recovery focused modality.
