Where the work happens
Case management services are provided to adults with psychiatric disabilities, some of whom are experiencing homelessness. These services include crisis intervention, information and referrals, housing assistance, benefits advocacy, employment assistance, support to appointments, and coordinated psychiatric care and therapy.
In 2015, TLCS in collaboration with sacramento county department of behavioral health services launched the triage navigator program. TLCS navigators are embedded within local hospital emergency rooms including sutter general, mercy general, methodist hospital, uc davis, mercy san juan and kaiser south.
Residential services include interim, and permanent supported housing for individuals with psychiatric disabilities. Specialized housing programs are available for individuals with secondary challenges including homelessness, AND/OR subtance abuse.
