Avenidas Rose Kleiner Center
Adult day health care provides transportation, nutrition, recreation, social service, and health services (including nursing care, physical therapy, occupational therapy, dietary and speech therapy) to frail seniors and dependent adults. The avenidas rose kleiner center had 13,175 days of service provided to 141 unduplicated clients.
Participant & Health Services
Provides reception, registration, membership to older adults, and facilitates the provision of education, recreation, and social services. 8,296 registrations were provided to 1,687 participants and health and wellness services had 6,467 service units provided to 1,115 clients.
Avenidas Village
Provides social, supportive, practical and other services to older adult members in support of maintaining their independence in their own homes. The village serves 265 members. Even with the COVID-19 pandemic, service and connections continue to be made through remote offerings.
