Transitional Housing Program
This program provides temporary housing for approved individuals suffering from a mental health illness or drug and alcohol addiction. Population includes those with a criminal justice history as well as those moving from a restrictive inpatient or residential setting.
Community Based Care Management Program
Breaking the barriers critical responsibility is to cultivate housing opportunities and to assist participants in maintaining their current housing, avoiding homelessness, or transitioning from homelessness to a more appropriate stable housing.
CC mobile crisis services is a program that provides immediate support to individuals who are experiencing a mental health crisis or behavioral health emergency
