Home and Community-Based Services to the Elderly

Case management, case aide, companionship, congregate meals, emergency alert response, personal care, nutrition education, nutrition counseling, outreach, transportation, energy assistance, health support, housing improvement, medical supplies, material aid and pest control.

Home Delivered Meals

Provides nutritionally-sound meals for eligible persons. Emphasis is placed on serving elders who are at greatest economic and social need, low-income minorities and those who are at nutritional risk. During 2024, a total of 79,296 frozen home delivered meals were provided.

Respite

In-home respite care is the provision of relief or rest for a primary caregiver from the constant, continued supervision and care of a functionally impaired senior by providing care for the person in the home for a specified period of time. During 2024, 17,340 hours of respite were provided.
