Long Term Care Ombudsman

Under contract with county area agencies on aging, we continued providing LTC ombudsman service to those in long term care in six area counties. Our emergency coverage agreement with bradford, sullivan, susquehanna and tioga counties was activated as they experienced turnover.

Guardianship

Court appointed guardians play a vital role in assisting those in a state of cognitive decline to make decisions about their lives. Guardians focus on enhancing quality of life , preventing exploitation, ensuring access to community services, and providing financial management.

Patient Advocacy

Our patient advocates continued to collaborate with hospitals and other health care providers to assist recently discharged older patients achieve their discharge goals. Most clients have chronic or complicated conditions.
