Care Coordination/Community Service

Our care coordinators served 4,125 clients and helped committed patients navigate a complex health care and social service organizations to address doctors and providers willing to provide the services they need. We reduced the cost of the care for our partners by increasing access.

Designed to help uninsured and under insured individuals gain access to the right mental health professional to meet their needs.

With partner support, we purchase health insurance on the Exchange for those who cannot otherwise afford the premiums. This allows patients to access ongoing, appropriate health care rather than emergency care.
