The org's own posting, as writtenA Day in the Life You will coordinate and facilitate the admission process, including referral review, pre-admission screening, communication with referral sources, resident and family education, completion of required documentation, and accurate entry of resident information into the electronic medical record. Collaborate with interdisciplinary team members, providers, residents, and families to support effective care planning, continuity of care, and achievement of resident-centered goals. Coordinate discharge planning and care transitions by arranging post-discharge services, equipment, follow-up appointments, and communication with external healthcare providers and agencies.
Maintain accurate resident records and provides administrative support through documentation management, benefits assistance, data entry, and related operational functions. Participates in quality assurance, performance improvement, and regulatory compliance activities to promote operational excellence, resident satisfaction, and positive clinical outcomes.
