This role combines primary care and addiction medicine, providing assessment, diagnosis, treatment planning, medication management, and ongoing care coordination. The Nurse Practitioner works collaboratively with a multidisciplinary healthcare team to promote continuity of care, support positive health outcomes, and ensure high-quality services for the individuals we serve.
