What You'll Do

Be a voice for residents. Advocate for their rights, dignity, and well-being. Build meaningful relationships. Connect with residents, families, and care teams. Make transitions easier. Guide residents and families through move-ins, care planning, and changes in care. Lead with compassion. Provide emotional support, resources, and solutions. Make an impact. Coordinate personalized care plans, assessments, and social services while ensuring regulatory compliance.

What You Bring

Master's in Social Work + 1 year of healthcare experience OR Bachelor's in Social Work, Psychology, Sociology, or related field + 3 years of healthcare experience At least 1 year of supervised healthcare experience Social Worker license as required by state regulations CPR certification A passion for helping older adults live their best lives!
