This role supports individuals transitioning from incarceration in state prisons, county jails, back into the community. The Medical Case Worker II works with clients during incarceration and post-release to ensure continuity of Medi-Cal coverage, access to comprehensive health services, medications, and supportive resources, including linkages to Enhanced Care Management (ECM). The position plays a critical role in improving health outcomes, supporting successful reentry, and reducing recidivism through coordinated, client-centered care.
