MJHS companies are qualified employers under the Federal Government’s Paid Student Loan Forgiveness Program (PSLF) Responsibilities Our MJHS Medical Associates, P.C. is a group of Nurse Practitioners, Physician Assistants, RN Case Managers and LPNs who provide care to Elderplan members who are residents of assisted living and long term care facilities, as well as to those living at home. In collaboration with physician, the NP will be providing comprehensive assessment, including multifactorial complexphysical examination and psychosocial needs, self - management education & tools to better manage patient’s multiple conditions.
As a part of Interdisciplinary Co-Management Program, the NP will be providing and coordinating primary care services to an assigned the at risk panel of Elderplan members who reside in the community. The NP will insure development of care planning and access to preventive care services in accordance with member and Plan health outcome goals and objectives. The NP will act as clinical resource to the member, family and interdisciplinary team.
Qualifications
