We are seeking a compassionate and skilled Primary Care Nurse Practitioner to join our medical group on a part time basis. The ideal candidate will provide comprehensive primary care services to our senior residents, ensuring they receive the highest quality of medical care in our sub-acute, long term care, assisted living, and medical office settings.
This role involves collaborating with the physician other healthcare professionals to develop and implement individualized care plans, promoting wellness, and managing chronic conditions. The candidate will be expected to work three days at Applewood Village Freehold, NJ. We offer a very supportive work environment that includes a team of well-qualified medical, nursing, and multi disciplinary professionals.
Essential Functions
Conduct thorough assessments and physical examinations of senior residents. Diagnose and manage acute and chronic health conditions. Develop, implement, and monitor individualized care plans. Prescribe medications and treatments as necessary. Provide health education and counseling to residents and their families. Collaborate with physicians, nurses, and other healthcare team members. Maintain accurate and up-to-date medical records. Participate in care team meetings and contribute to quality improvement initiatives. Ensure compliance with all regulatory and organizational policies and procedures.
Requirements
Master's degree in Nursing from an accredited program. Current Nurse Practitioner license in New Jersey. Certification as a Family Nurse Practitioner (FNP) or Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP). Minimum of 2-3 years of experience in a sub-acute or acute care setting. Medical office experience is a plus. Excellent clinical skills and knowledge of geriatric care in post acute settings. Strong communication and interpersonal skills.
